Change Member Information

To add an account, change your contact information, add or remove a joint or perform other account maintanance tasks, please complete the Change/Add/Delete Authorization form.

Instructions:

1. Download and complete the Change/Add/Delete (CAD) Authorization form. The CAD Authorization form can be filled out electronically. Be sure to fill out all applicable information, including an original signature.
2. Fax the form to the Member Service Center at 214.880.9501 or mail form to the Member Service Center at:
